- Information return on releases of refined sugar by the proprietor or operator of a sugar refinery or mill for June 2017
- Copy of the list submitted to the nearest DOLE Regional/Provincial Offices – Operations Division/Unit showing the names of MWEs who received hazard pay, including justification for payment of hazard pay for the period of June 2017
